A tear or crack in the moist tissue at the anal opening, common in infants and adults
Causes include:
Passing hard or large stools
Straining during bowel movements
Diarrhea, childbirth, anal intercourse
Inflammatory bowel disease, infections, or less often, tumors
Persistent muscle spasm of the sphincter reduces blood flow, preventing healing and often turning acute fissures into chronic ones
Sharp, burning pain during and hours after defecation
Bright red bleeding—on stool, toilet paper, or in the bowl
Visible tear, sometimes with a skin tag (“sentinel pile”) in chronic cases
Muscle spasm with a feeling of tightness in the anus
Based on symptoms and physical exam
Often diagnosable visually, sometimes aided by gentle anoscopy or palpation under anesthesia .
High-fiber diet (25–35 g/day) + hydration
Warm sitz baths (10–20 min, 2–3× daily) .
Stool softeners and gentle hygiene (no harsh wiping)
Topical nitrates (nitroglycerin) or calcium-channel blockers (diltiazem/nifedipine) to relax the sphincter
Topical lidocaine for pain relief
Botox injections into the sphincter muscle, offering healing rates of ~60–80%
Lateral Internal Sphincterotomy (LIS): A small cut in the internal sphincter to reduce pressure. Heals ~95–98% in ~3 weeks; low risk of incontinence
Alternatives: Advancement flap or fissurectomy, though LIS is preferred .
Keep stools soft: Fiber, water, stool softeners
Avoid straining; respond promptly to the urge
Gentle toileting and good hygiene
Pain or bleeding persists after a few weeks of self-care
You suspect a chronic fissure or experience incontinence/persistent symptoms

An anal fissure that heals within six [6] weeks is acute and anal fissures that last for six [6] weeks or more are called chronic anal fissures. Acute fissure can be treated with conventional treatment, but chronic fissure may require surgical intervention.
An anal fissure is a small tear or cut in the lining of the anus. Hemorrhoids are a group of inflamed and swollen tissues in the anal area. If you suspect having any of these anorectal diseases, get consulted by the best proctologists at your nearest location.
If anal fissures are left untreated, they can cause severe constipation, fecal impaction, pain in the anal area, sentinel pile, difficulty to do daily activities and can form an anal fistula. To avoid these, you should not delay the anal fissure treatment.
Regardless of the severity of your anal fissure, surgery is considered to be the permanenet cure for anal fissure. In case of anal fissure surgery, the chances of succes is much higher than open surgical treatment.
